Notes:

  • The dates indicated above mean that the ward or city will begin sending forms from the date or period specified onwards. This means that Late May could extend to early June in some cases.
  • The application forms will be addressed to the head of the household, who can then apply for all members in one form.
  • Most of the wards and cities have indicated that they will be accepting applications until mid-August.
  • As with all official forms in Japan, the application sheet is in Japanese. A translation of the model sheet can be found here.

Online Applications for the Coronavirus Payout

Alternatively, you can apply for the cash payout via the My Number Portal Site. Some municipalities have begun accepting online applications as early as May 1st. The website is in Japanese only and you will need the following to apply:

  • My Number Card
  • Your PIN and password for My Number (if you get this wrong 5 times you will be locked out of your account)
  • My Number App for Desktop/Mobile
  • Your bank details
  • Proof of bank account (photo of cash card, bank statement, etc.)

During the process, you will be asked to scan your My Number card. I used my smartphone to apply and the instructions I got were to place my phone in the middle portion of my My Number card. This worked just fine in the early parts of the process but kept returning “Scan Failed” at a specific portion of the application. Unfortunately, I got locked out on my 5th attempt, so I’m instead waiting for the form that will arrive by post.

    • Thank you for pointing that out. I’ve just added Shinagawa. Unfortunately, they have not specified any dates on their website.

      • I got so close, bought an IC Card reader and at the final stage of the application was told the digital certificate had expired. The certificate should last 5 years and mine is only 2 years old.

  1. Completed the online form today using my phone: what a hassle! It took 30 minutes of trying before I saw the note that said to use the Chrome browser! My default browser is Firefox and I kept getting a scanning error on the signature page! After switching to Chrome, I finally got it to work!

When scanning your My Number card, the gold chip side should be facing the phone, not the photo side.

And, when photographing your bank card or bank book, make sure the resolution is not too high. The site recommends a max height size of 1920 pixels.

    Good luck y’all!

  2. What is the website so that we can download the form. I leave in nerima and it says form can be downloaded at the website.what is the website please.

      • When you click Nerima on the table above, it’ll take you to the ward’s official page. Scroll down to see the PDF link for the form as well as the sample reference. The address is also written there.

  3. I used my phone to register because I don’t have a reader for my PC. I barely had to input any info because after successfully scanning the MyNumber card, the textboxes filled themselves automatically.

I got scared when they asked my PIN because I’m bad at remembering stuff I never use. Fortunately, my past self was smart enough to use something I’d try to punch in now… and I got in. The problem was when it asked my password. I tried the passwords I commonly use, nada. I tried it 3 times. Anyway, I’ll just try and change my password tomorrow and continue my registration.

    • UPDATE: Since our MyNumber cards are nearing their expiration date, we decided to go to the city hall and renew them. Process went smoothly and they got extended again.

Anyway, I asked them about my password and the problem I have. Apparently, when we moved houses a couple years ago, the password got reset along with it. So we registered a new password. After a couple minutes, we got our cards back.

I went to the online portal and continued my online registration. It worked! Now I got my application number and I’m hoping the money gets transferred next week.

  4. I already downloaded the form. After I fill out the form what is the next step? Do I need to send it to post office?

    • Are you referring to Nerima Ward’s form? Once you’ve filled out the form, send it by post. The address is written on the same page as where you’ve downloaded it.
