Sending of Vaccination Tickets

| Above 75 (Born before April 1, 1947) | April 26th (Mon) |
| 65-74 (Born between April 2, 1947 and April 1, 1957) | May 20th (Thur) |
| 60-64 (Born between April 2, 1957 and April 1, 1958) | June 10th (Thur) |
| 16-59 (Born between April 2, 1958 and May 31, 2005) | June 17th (Thur) |
| 12-15 | July 15th (Thur) |
Tickets for vaccination will be sent by postal mail.

Reissue of Vaccination Tickets
You may apply to have your vaccination ticket reissued in case of any the following:
- loss or damage of the vaccination ticket
- change in address after the vaccination ticket has been issued
- use of pre-examination slip twice
To apply for reissue, you must fill out the application form (Word file) and send it to the following address:
