Tickets for vaccination will be sent by postal mail.

Materials Enclosed

The following will be included in the postal mail that will be sent by Shinjuku Ward.

  2. 3 copies of the pre-examination slip
  3. Instructions for vaccination
  4. Vaccination notice
  5. Reservation process

Reissue of Vaccination Tickets

You may apply to have your vaccination ticket reissued in case of any the following:

  • loss or damage of the vaccination ticket
  • change in address after the vaccination ticket has been issued
  • use of pre-examination slip twice

To apply for reissue, you must fill out the application form (Word file) and send it to the following address:
