  • The dates indicated above mean that the ward or city will begin sending forms from the date or period specified onwards. This means that Late May could extend to early June in some cases.
  • The application forms will be addressed to the head of the household, who can then apply for all members in one form.
  • Most of the wards and cities have indicated that they will be accepting applications until mid-August.

Online Applications for the Coronavirus Payout

Alternatively, you can apply for the cash payout via the My Number Portal Site. Some municipalities have begun accepting online applications as early as May 1st. The website is in Japanese only and you will need the following to apply:

  • My Number Card
  • Your PIN and password for My Number (if you get this wrong 5 times you will be locked out of your account)
  • My Number App for Desktop/Mobile
  • Your bank details
  • Proof of bank account (photo of cash card, bank statement, etc.)

During the process, you will be asked to scan your My Number card. I used my smartphone to apply and the instructions I got were to place my phone in the middle portion of my My Number card. This worked just fine in the early parts of the process but kept returning “Scan Failed” at a specific portion of the application. Unfortunately, I got locked out on my 5th attempt, so I’m instead waiting for the form that will arrive by post.
